Consent to the processing of personal data (medical tourism)

I, the subject of personal data, hereby voluntarily, of my own free will, and in my own interests, in accordance with Federal Law of the Russian Federation No. 152-FZ of July 27, 2006, “On Personal Data,” give my consent to the State Budgetary Healthcare Institution “V.M. Buyanov City Clinical Hospital of the Moscow Health Department,” Taxpayer Identification Number (INN) 7724598966, registered in accordance with Russian legislation at the address: 26 Bakinskaya St., Moscow, 115516 (hereinafter referred to as the “Operator”), which owns the website https://gkb-buyanova.ru (hereinafter referred to as the “Site”) (the Operator’s resource through which personal data is processed) to process my personal data of a different category (not related to special categories of personal data or biometric personal data), as specified by me by completing the web form on the Operator’s Website, namely:

– Name;
– Phone number;
– Email address;

For the purpose of reviewing an application addressed to the Operator

by collecting, recording, systematizing, accumulating, storing, using, clarifying, retrieving, blocking, deleting, or destroying data, carried out both without and with the use of automated tools.

1. This Consent is valid until the purpose is achieved. The personal data storage period is 1 year.
2. Personal data may be processed only by the Operator, without outsourcing the processing to other legal entities or individuals.
3. This consent may be revoked by me or my representative by sending a written request in any form or an electronic request signed in accordance with Federal Law No. 63-FZ “On Electronic Signature” of April 6, 2011, to the Operator at the following address: 115516, Moscow, st. Bakinskaya St., 26, or by email at inter@gkb-buyanova.ru
4. The Operator will cease processing personal data within 30 days from the date of revocation, and will send written notice to the subject.
5. The Operator has the right, after receiving the revocation of this Consent, as well as after the expiration of this consent, to continue processing my personal data to the extent that consent is not or will not be required for such processing under current Russian Federation law (Clause 2, Article 9 of Federal Law No. 152-FZ of the Russian Federation “On Personal Data” of July 27, 2006).
6. Personal data will be destroyed in the manner and within the timeframes established by Article 21 of Federal Law No. 152-FZ of July 27, 2006 “On Personal Data.”
