Consent to the processing of personal data (medical tourism)

I, being a legally competent person, voluntarily, of my own free will and in my own interests, on the basis of the Federal Law of the Russian Federation of 27.07.2006 No. 152-FZ “On personal data” and the Federal Law of the Russian Federation of 02.05.2006 No. 59-FZ “On the procedure for considering appeals of citizens of the Russian Federation” give my consent to the State Budgetary Healthcare Institution “City Clinical Hospital named after V.M. Buyanov of the Department of Health of Moscow”, INN 7724598966, registered in accordance with the legislation of the Russian Federation at the address: 115516, Moscow, st. Bakinskaya St., 26 (hereinafter referred to as the “Operator”), which owns the website https://gkb-buyanova.ru/ (hereinafter referred to as the “Site”) (the Operator’s resource through which personal data is processed) to process my personal data of a different category (not falling under the special category of personal data or biometric personal data), namely:

• Full name
• Telephone number
• Email address

for the purpose of considering a citizen’s request

through collection, recording, systematization, accumulation, storage, clarification, retrieval, use, and destruction, carried out both without the use of automated means and with their use.

Acceptance of the Consent offer is indicated by checking the box “I consent to the processing of personal data.”

This Consent is valid until the purpose is achieved. The personal data storage period is 1 year.
Personal data is processed only by the Operator, without the assignment of processing to other legal entities or individuals.

This consent may be revoked by you or your representative by sending the Operator a written or electronic request, signed in accordance with Russian legislation on electronic signatures, to the address specified at the beginning of this Consent.

Personal data will be destroyed:

• upon achievement of the personal data processing objectives;
• upon liquidation or reorganization of the Operator;
• based on a written or electronic request from the personal data subject, signed in accordance with Russian legislation on electronic signatures, to cease processing their personal data.

The Operator will cease processing such personal data within 30 days from the date of revocation, and will send written notice thereof to the subject.

The Operator has the right, after receiving the revocation of this Consent, as well as after the expiration of this consent, to continue processing my personal data to the extent that Consent is not or will not be required for its implementation by virtue of the current legislation of the Russian Federation (clause 2 of Article 9 of the Federal Law of the Russian Federation of July 27, 2006 N 152-FZ “On Personal Data”).
